The Arc Dental Program

Welcome to the Arc of Florida Dental Program

                                             PLEASE READ BEFORE APPLYING FOR FUNDING PROGRAM

ELIGIBILITY:
In order to qualify for The Arc of Florida Dental Program services, individuals must receive services through the Agency for Persons with Disabilities (APD): WAIVER, WAITLIST, or CDC+ programs. To enroll with APD for these services, call 1-866-273-2273.

Individuals who are not enrolled with APD, should NOT submit an application to The Arc of Florida Dental program.
CURRENT DENTAL FUNDING PERIOD: July 1, 2021-June 30, 2022 (Dental Services approved by The Arc of Florida Dental Program are contingent upon the availability of State Funding. The period may end earlier if all funds have been spent.)

IMPORTANT: The Program funds current treatment, not future treatment. Based on scope and cost, all current treatment may not be funded within a Fiscal Year. Funding for Phased Treatment (due to high cost) does not guarantee continual funding in any subsequent Fiscal Year. Funding for Full Mouth Extractions does not guarantee funding for Replacement Appliances.

APPLICATION PROCESSING: Applicants must reapply every year on or after July 1st. Applicants’ funding for services is not continual within a contract year. Submission of application does not guarantee approval for funding. Due to the high volume of application submissions, it may take more than 30 days to process/make determinations on applications and requests for receipt or status of application, whether or not an application is complete, and/or timeline for receiving funds cannot be fulfilled.
When a determination has been made, we will notify the Primary Contact or Waiver Support Coordinator listed on the application. If there has been no notification by our office then a determination on the application has not been made and the applicant should continue to wait for a decision.

HOW TO APPLY FOR FUNDING: 

*Please only submit one complete application per person.
Incomplete applications (i.e. missing signatures, contact information, Network Dentists selected, Treatment Plans/New Patient Estimates) will not be accepted and cannot be processed.

1. Before Downloading the application, please review the full instructions located on The Arc Dental Program main information page.

2. Complete Page 1, complete and sign Pages 2-4 of the Application Packet.

3. Select a Program Network Dentist (if you do not have one) from the Arc of Florida Dental Office List (CLICK LINK BELOW) that meets all needs of the applicant (dentist type, sedation, location).

*Please carefully select Dentists for Applicants. Approvals cannot be revised and an applicant is funded for one Treatment Plan without revision per contract year.

WRITE THE NAME OF THE SELECTED DENTIST ON THE APPLICATION (ON THE LINE CURRENT DENTIST).

When making a selection, please pay close attention to whether or not:

– Dentists are General Dentists (can be selected by any applicant)
– Dentist are Specialists (must first be seen then referred by a General Dentist) such as, Oral Surgeons, Endodontists, Periodontists, Prosthodontists – Dentists perform the type of Sedation needed by the applicant
– Dentists are located in or near the applicant’s county of residence

4. Contact the selected Dentist, request and obtain a New Patient Estimate (if you are not a current patient of the Dentist) or Treatment Plan (if you are a current patient of the Dentist). Also, ask the Dentist if your treatment will be covered by your Dental Insurance and/or a Medicaid Managed Care Dental Plan (i.e. Liberty, DentaQuest, MCNA). If some or all of your treatment is not covered, CHECK THE CORRECT OPTION, ON THE APPLICATION, ANSWERING THE QUESTION ABOUT WHY YOU ARE UNABLE TO USE YOUR INSURANCE/MEDICAID MANAGED CARE DENTAL PLAN (i.e. Denied Treatment, No Available Provider, Not Enrolled in Insurance/Plan).
*Not required but if available, please include Denial Letters with Application Packets.

5. Return by Email, Fax, or Mail to our office THE COMPLETED AND SIGNED APPLICATION PACKET (Pages 1-4-do not return the Notice of Privacy Practices for Protected Health Information-keep for your records), NEW PATIENT ESTIMATE or TREATMENT PLAN, and A COPY OF THE DENIAL LETTER (if available):
